General office policies and information:
- We do require 48-hour notification for cancellation of any appointment. Please call if you need to reschedule or cancel an appointment – we will do our best to accommodate your request in order to avoid a late cancellation fee. We charge in-full for missed appointments or late cancellations
- The Clinic does not participate in any “commercial” insurance plans; PAYMENT IS REQUIRED at the time of service. We provide an invoice to submit to your insurance plan. We will assist you in any way possible when submitting your claims for reimbursement, and we will do our best to help with pre-certification of services such as a Letter of Medical Necessity, or “doctor-to-doctor” phone calls. We do not participate in Medicare or Medicaid or any insurance affiliated with the U.S. Military, Department of Homeland Security, or any Federal or State government programs.
